Bit of a problem with yr semantics .. Kyu Sakamoto died in the August 12, 1985 crash of JAL Flight 123, a Boeing 747 on a scheduled passenger flight from Tokyo to Osaka, that suffered structural damage - the pilot had reported a problem with a rear door - and decompression shortly after take off, and which remains the deadliest single aircraft accident in aviation history. Prior to that accident the aircraft was involved in a tail strike that damaged the aft bulkhead. Inadequate repairs were done and it was certified safe to fly when in fact it wasn’t. The bulkhead failed on that flight.
