Here's a story that goes around:

Helicopter pilots have a final pre-flight checklist that they must perform before taking off. This one pilot never did the list, putting the burden of final inspection on the ground crew.

One day as he's lifting off, a ground crewman runs on the pad jumping and shouting and waving a Jesus nut around in his hand.

Pilot shits his pants (thinking it's HIS Jesus nut) and does his own preflight from then on.

That's how I remember the story.

Preflight inspections are required. They actually have to do paperwork ensuring they do it.

Yes, but a pilot can still take off without one.

I regularly watch a couple of flight debrief aircraft crash ytube channels - it is unbelievable how many accidents (GA, commercial, Military) also have pilots not doing pre-flights or running an in flight check list.
