2008 Wesfarmers coal mine crane collapse

A crane-related incident occurred at Wesfarmers' Curragh coal mine in central Queensland on December 8, 2008. During scheduled maintenance of one of the mine's five draglines, a winch failure caused the dragline's boom to fall approximately 30 meters. The incident injured the crane operator, who sustained minor injuries. The accident also caused significant structural damage to the dragline's boom. Mine management, along with independent assessors and government work safety authorities, launched a full investigation into the cause of the incident. Wesfarmers stated that an estimate of the repair costs and the impact on the mine's operations would be provided when available.

One of the craziest ones I've seen.

I remember it well, we were analyzing this disaster, in short the head crane (boom head) was poorly under capacity with not enough boom...the heel crane (directly in front of the derrick) had the capacity but was not configured properly nor was the rigging of the load...there should have been no jib on the boom and a multi spreader system should have been used to support it's share of the load...this was poorly managed and planned...

I've seen reports saying the operator in the dragline fucked up & put the cables in free fall, also another report saying the winch brakes failed. Afaik, it was never conclusive to what actually happened here.
