Multiple reasons.

1. Cremation has become the far cheaper option. And, it used to be a bit taboo from a Christian perspective ... but now it's very popular. (>60% today in the US.)
2. Military cemeteries have lots of room and all honorably discharged vets have the option of taking a free plot for themselves and their spouse. With WW2, Korea, Vietnam ... a notable % of the population (and their spouses) went there. (I know several living Vietnam vets who are expecting to be interned in a nearby national cemetery.)
3. Many rural cemeteries were over platted allowing for room to grow. Every once in a while, you will see them expand by moving fencing, etc.
4. Mausoleums (going vertical) are becoming more popular in some parts of the country.

Here in Northern Ontario, whomever decided the size of the cemetery when settlements where established 150 years ago, made them big enough to have plenty of room for decades to come.
In Germany, each grave site needs to be rented, and after a few years it gets reused. It is said that the cemetery in Cologne gets completely overturned about every 50 years (I have not confirmed this).

In my area there are lots of new cemeteries, but, they start off as parks and take decades to fill in as the cemetery.

One has a petting zoo.
Another has a train.

It's not like the dead stay indefinitely at cemeteries. Once their families stop paying for the slot they get removed. I think you have to pay for the slot every 25 years or so.
