It depends on the state and they all vary, some don't require you to inform the officer, some don't require you to have a "concealed carry" permit.

There are some weird ones like Georgia where if you don't have a permit you can carry in your glove box but without a round chambered(revolvers exempt).

If you carry in the US you need to know so you don't violate their laws. Some states recognize other state permits, some don't. Some states allow their residents to carry unrestricted but don't allow non residents to.

The first two you listed are how guns work. Chambered round, safety off. Otherwise it’s useless in self defense. That’s the way I carry all my pistols (except my 1911, I keep it safetied.) The no holster is debatable. Some pocket pistols don’t require holsters.

Your situational awareness is what will save you nearly every time. You are failing if you allow a situation to progress to the point that you need to quickdraw with a chambered round, and safety off.

I have no problem carrying with a round in the chamber and safety off. Some situations happen despite your awareness. I would say his biggest mistake was not informing the officer that there is a round in the chamber and the safety is off. That may have made her more careful and handle it better, or call someone with better training to retrieve it.

Yup. I have practiced drawing and thumbing the safety in one motion so it's not even a loss of time. The act of pushing down on the top/side of the holster while drawing flips the safety on my holster/gun so it's just about the training.
