Curious of the final report, what was the actual cause - human error (careless high school operators) failure to secure harnesses?, or equipment malfunction.
From what i remember - clips at the time showed the harness not fully locked in place on lil gargantuan - which should have precipitated a red light on the control board before someone pressed the big ole get wreckt activation button.
