Multiple shots fired in a crowd full of people. No one gets hit, except Trump and he gets supposedly hit on the ear.
Before he falls to the ground, he touches his ear but there's no blood on his hands. Then multiple secret service members jumps on top of Trump, then next time we see him, there's supposedly blood on his ears and face. This is all based on the video footage I saw.

THEORY: Shot's fired in the distance, Trumps plays along as if he got hit on the ear, and intentionally drops on the floor to position himself where the camera can't capture him, secret service surrounds his body, covering it from the nearby audience, allowing them to add some fake blood to his ears and face without anyone seeing, so that when gets up, he has blood on his ears and face, the cameras are all ready and in position to take the 'iconic photos' you see below
