I started suffering from significant depression and other mental health issues, with alcohol problems added in.

The only one preventing him from earning a livelihood is him. Instead of doing that, he's acting like a beggar.

As for his looks, I think we can all acknowledge he's never going to make it on only fans. But maybe he could have a business installing fans, HVAC, or whatever?
