They are injecting enough of the population to continually adjust the demographics of any area in the world using injections that cause infertility and/or early death.

They don't want to kill every man, woman, and child.
They don't poison every injection. They poison a percentage of injections depending on the demographics of an area and how quickly they want to change that area's demographics.
They are able to control the birth and death rate of the world through infertility and early deaths. But they are targeting specific areas, subtly adjusting the demographics of those areas over the course of decades.
By keeping the numbers low enough each year, they are able to create doubt and deniability.

Again, they are not poisoning every injection. They do not want to kill every man, woman, and child. Instead, they have set up a system that allows them to change the demographics of any area in the world whenever they want.
