Some of the injections were safe, while a specific percentage were poisoned.
They are not trying to kill every man, woman, and child.

Instead, specific percentages of poisoned injections are given to different areas in order to strategically thin out selected populations and slowly change the demographics of certain areas by causing infertility and sudden deaths.

By doing this over the course of a couple decades, they are able to keep the amount of deaths and infertility low enough each year to create doubt and deniability.

Some areas got a far higher percentage of poisoned injections than others. Due to them wanting to change the demographics of that area more rapidly.

Just because people aren't dropping dead in your area, that doesn't mean that they aren't in other areas.
