No, Rome had the most modern sanitation system in Europe at the time. You should read "An Underground Education" by Richard Zacks, a jew, who takes some enjoyment from describing how awful European cities in the last millennium must have smelled. He dwells on the French courts and their obsession with enemas and holding court on "the throne", i.e., on a toilet.

From my own research on Elizabethan London, most areas are cesspools because of inadequate drainage, and London was hardly unique.
