This is frequently a sign of an unhealthy repression of sexuality with no healthy ways to acknowledge that it is in fact a normal part of being a flesh bag called "human." In order to deal with that repression, one of the mind's slickest tactics is to internalize that attraction and turn it into a different desire: become the thing that is believed to be unobtainable or that self is somehow unworthy of having.

The absolutely sad thing here is that it wouldn't even take a month of therapy (just the talking kind) to work this out, figure out how to unblock that, and adopt healthy attitudes and move on from this. Sadly, the medical world will just hand this poor person estrogen and say "You woman now, you happy."
