This same scenario was the premise for an episode of some show - no idea what it was tho.
A family - mom, dad, teen daughter - go on a remote road trip and stop at a backwoods outhouse so the daughter can relieve herself.
Obviously she takes the phone with her, and she proceeds and then manages to drop the phone down the hole.
She then tries to go after it.
After a long while, the parents finally go to check on daughter, and surmise that she is inside the hole, and together they both also meet the same fate.
This was the first show that I have seen to actually highlight the dangers of fumes in an unventillated area. All three of the victims lost consciousness and died due to breathing noxious fumes.
One of the first things I learned about (and was constantly hammered into us) for working with large tanks was to always use breathing apparatus and to suppress the instinct to immediately jump after a co-worker who fell unconscious inside the tank.
