There are dudes who have a feeding fetish, where they find a fat woman and overfeed her until she can't get out of bed. That's usually what's at work in these situations, or you have a permissive family shoveling food into her gullet on a kind of Munchhausen's by Proxy trip. She's not fit enough to stand up on her own, so there's no way she cares for herself.
