Those letters aren't... verbose. The main pain is getting them sent. Stamps, etc. And that's the main deterrent, too. They can approve 90% of appeals because sufficiently few people make them. At some point, it costs them more to do paperwork than pay for your treament (which they pay much less than cash price for, so divide whatever you think it costs by 3).
I'd also be worried about the AI filling your letter with garbage. On the off chance anyone reads it, it'd be better if you just say what you want and why they should pay for it. I would pay someone to do the mechanics of it, though.
